Our Services
In home care services are chosen by the client and/or client’s family before services begin with a CHS supervisor. They are detailed in an Individual Service Plan. This plan is given to the assigned caregiver when services begin. A supervisor goes to the home on the first day of services and helps the caregiver inclement to the client’s home. For the first two weeks of services the supervisor is assigned to make sure the caregiver learns to do things the way the client and/or family wished them to do be done. This may involve home visits, phone calls, or meetings to perfect the plan and find solutions to problems. The client and/or family may stop services at any time with a 24 hour written notice by mail, or email. The client and/or family may request a new caregiver with no questions asked. The client and/or family may request a new Individual Service Plan at any time. At all times the client and/or family has the right to be 100% satisfied with the services received from CHS. Champion Homecare Solutions does not pretend to be a perfect agency, but we do promise to have integrity and do our best to meet your needs.
We follow a select and personalized Plan of Care utilizing participation of the client, family and/or caregivers. The recipients of our personal care can appreciate a composite of any of the following services:
